Fall Park Playdates!
Join us for a playdate in the park. Locations vary, but the community will stay the same.
Service Description
We are so excited to spend this autumn season with you. Below is the full list of FALL playdate locations and addresses to help you find the correct park each month. All playdates begin at 10:00 AM and include simple crafts, snacks, music, and community connection for moms and little ones. Feel free to bring water, sunscreen, blankets, or outdoor toys as desired. We can’t wait to see you this fall! September 25th at ValPlayso in Valpo October 16th at Prairie West Park, St. John November 4th at Destination Park in Hobart. Cancellations In the event of a scheduling conflict, please cancel your reservation asap so that another mom can sign up for this event. Thank you! Notice of Privacy Practices As a rule, the Northwest Indiana Center for Maternal Wellness will disclose no information about you without your written consent. There are some important exceptions to this rule of confidentiality. I may use or disclose records or other information about you without your consent or authorization in the following circumstances: · Emergency: If you are involved in a life-threatening emergency and I cannot ask your permission, I will share information if I believe you would have wanted me to do so, or if I believe it will be helpful to you. · Child Abuse Reporting: If I have reason to suspect that a child is abused or neglected, I am required by Indiana law to report the matter immediately to the Indiana Department of Social Services. · Adult Abuse Reporting: If I have reason to suspect that an elderly or incapacitated adult is abused, neglected or exploited, I am required by Indiana law to immediately make a report and provide relevant information to the Indiana Department of Welfare or Social Services. · Serious Threat to Health or Safety: Under Indiana law, if I am engaged in my professional duties and you communicate to me a specific and immediate threat to cause serious bodily injury or death, to an identified or to an identifiable person, and I believe you have the intent and ability to carry out that threat immediately or imminently, I am legally required to take steps to protect third parties. These precautions may include 1) warning the potential victim(s), or the parent or guardian of the potential victim(s), if under 18, 2) notifying a law enforcement officer, or 3) seeking your hospitalization. By my own policy, I may also use and disclose medical information about you when necessary to prevent an immediate, serious threat to your own health and safety. Confidentiality In reserving your spot at this event, you are agreeing to maintain confidentiality for all attending group members. You will not share identifying information to anyone outside of this event, respecting the confidentiality and privacy of everyone present. You acknowledge that this event is a safe space for mothers to share about their life experiences and that you have a role in maintaining this safety. Thank you for your consideration.
